  • Providedirect support and case management for individuals in addiction recovery.
  • Conductclient assessments, develop care plans, and track progress.
  • Connectclients with essential services such as housing, employment, andhealthcare.
  • Coordinatewith medical professionals, counselors, and community resources.
  • Maintainaccurate documentation and ensure compliance with regulatory guidelines.Qualifications:Required:
    • Bachelor'sdegree in Social Work, Human Services, Psychology, or a related field.
    • 1+years of experience supporting individuals with substance use, mentalillness, or homelessness.
    • Strongproblem-solving, organizational, and communication skills.
    • Flexibilityto work variable shifts.Preferred:
      • Master'sin Social Work (MSW) or Social Work Certification (LICSW, LSW, LCSW).
