Comfortable working with families and planning for long-term care placement. Description: 2 years of experience in a hospital setting is required. An active compact license is mandatory. BLS certification is required. The case management team consists of social workers covering social work consults within the hospital. The patient ratio is between 25-40. The department includes a manager and a director. Documentation is performed using specific computer systems. The dress code is business casual. No weekends are required. Experience in inpatient social work is important for the role. Additional qualifications include skills in discharging and planning for long-term care. Unit-specific guidelines and policies will be provided during onboarding.
